01Overview
What pyometra is, and why it cannot wait
After every season, hormones thicken the lining of the womb. Over the years the lining forms cysts, bacteria from the vagina settle in, and the womb fills with pus. About 1 in 4 unspayed bitches develops it by the age of 10. Cats get it too, though less often.
There are two forms. In open pyometra the cervix is open and you see a cream, brown or bloody discharge. In closed pyometra nothing drains. The womb swells, toxins reach the blood and the kidneys, and the womb can rupture. Closed cases look less dramatic at home and are more dangerous.
The reliable cure is surgery to remove the womb and ovaries. It is the same operation as a dog spay or a cat spay, but on a sick patient with a fragile womb. That makes it longer and more costly than a routine spay. The price is quoted in writing after the exam.

Examination
On arrivalThe vet checks her temperature, gum colour and hydration, and feels the belly gently. A swollen womb is never squeezed. You are asked about thirst, vomiting, discharge and any hormone injections.

Blood tests
Blood is run in our lab while you wait. A high white cell count supports the diagnosis. Kidney values, blood sugar and salts show how sick she is and how the anaesthetic should be planned.

Ultrasound
A healthy womb is hard to see on a scan. A womb with pyometra shows as fluid-filled loops. Ultrasound also rules out pregnancy, which can look similar from the outside.

Drip and written quote
She goes on a drip with pain relief and antibiotics by injection. You receive the cost in writing before surgery, with the anaesthetic, the operation, the nights in the clinic and rechecks listed.

Surgery
Usually the same dayOnce she is stable, the womb and both ovaries are removed under general anaesthetic. A nurse monitors heart rate, breathing and oxygen throughout.

Recovery in the ward
Most patients stay 1 to 2 nights on a drip. She goes home when she is eating, her temperature is normal and the blood values are moving the right way.

Injections and pills to stop heat
Hormone products that suppress a season are still sold and used in the region, mostly in cats. They are a recognised cause of pyometra, including in young animals. Tell the vet if she has ever had one.
Seasons all year
Cats here cycle in every month because there is no real winter. More seasons mean more chances for the womb lining to change. Unspayed indoor queens are the typical feline case.
Thirst that gets blamed on the heat
Drinking far more than usual is an early sign. In July it is easy to put that down to the weather. If the water bowl empties twice as fast and she had a season in the last 2 months, call.
Breeding females
Many owners here keep a female unspayed to breed her once. Each empty season raises the risk. If you no longer plan a litter, spaying removes the risk completely.
Travel and boarding
A season just before a summer trip means the risk window falls while you are away. Tell the boarding staff or the sitter the date, and what to watch for.

Come in the same day if you see any of these
In an unspayed female of any age, especially in the 2 months after a season.

- Pus, brown fluid or blood from the vulva, or constant licking under the tail
- Drinking and urinating much more than usual
- Refusing food for a day, with or without vomiting
- A swollen or painful belly
- Dull, slow to get up, or a high temperature
- Pale gums, collapse or cold legs. This is an emergency at any hour
05Afterwards
The first ten days at home
She has had major surgery while unwell, so recovery is slower than after a routine spay. Expect her to be quiet for 2 to 3 days. Appetite normally returns within 48 hours.
Antibiotics and pain relief continue at home. The recheck is usually at 3 days and again at 10, and the vet may repeat the kidney values.
- Finish the full course of tablets, even if she looks well
- Collar or body suit on until the wound is checked
- Lead walks only for dogs, and no jumping for cats, for 10 days
- Thirst should fall back to normal within a week. Tell us if it does not
- Call the same day for vomiting, a wet wound or refusing food again
